If you are a former patient of Dr. Swinamer and you would like to obtain a

copy of your medical records, please contact the

office by phone and staff will explain the procedure to you.

8:30 am -12:00 pm

Tuesday
8:30 am - 3:30 pm

Thursday
8:30 am - 4:30 pm

Friday
8:30 am - 12:00 pm
